Job Title: Clinical Team Lead RN - Home Health

At Enhabit Home Health & Hospice, we're committed to providing exceptional care to our patients in the comfort of their own homes. As a Clinical Team Lead RN, you'll play a vital role in ensuring the highest quality of care is delivered to our patients.

Responsibilities:
  • Assist the Branch Director in coordinating home health services to meet the needs of our patients.
  • Review and approve patient information submitted by Licensed Professionals during Start of Care, Recertification, Resumption of Care, or Evaluation visits.
  • Provide oversight of patient care and assist with case conferences, reviews, and approvals.
  • Assist the Branch Director with day-to-day office and staff management related to patient care.
  • Contribute to the overall company success of providing A Better Way To Care by effectively facilitating relationships between physicians, referral sources, patients, caregivers, and employees.
Qualifications:
  • Graduate of an accredited school of professional nursing and licensed in the state of employment and where practicing.
  • A minimum of two years nursing experience and one-year experience in home health.
  • A good understanding of Federal, State, and local laws/regulatory guidelines governing the operation of a Home Health agency.
  • Skilled competency with technology, including general office equipment and EMR platforms.
  • A valid state driver's license and automobile liability insurance.
