Job Summary:

The Registered Nurse will be responsible for delivering comprehensive end-of-life care to patients and their families, working closely with interdisciplinary teams to optimize clinical management and discharge planning.

Responsibilities:
  • Provide direct or indirect skilled comprehensive nursing care to patients in their homes or inpatient settings.
  • Conduct clinical assessments and evaluations to determine the level of care required.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare teams to ensure seamless transitions of care and optimal patient outcomes.
  • Document patient information and progress in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Completion of an accredited RN education program with a diploma, associate's or bachelor's degree.
  • At least 1 year of experience in hospice, home care, and/or community health.
  • Valid driver's license and ability to travel to patient locations.
  • Ability to work independently with minimal supervision and as part of a team.
  • Excellent communication and interpersonal skills.
