Job Title: Home Health Nurse

VNA Health Group is seeking a skilled Home Health Nurse to provide high-quality patient care in the comfort of their own homes.

Job Summary:

The ideal candidate will possess excellent communication skills, a strong clinical background, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Assess and evaluate patient needs, developing individualized care plans to meet their unique requirements.
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care.
  • Document patient progress, identifying areas for improvement and implementing changes as needed.
  • Maintain accurate records, adhering to agency policies and regulatory requirements.
Requirements:
  • RN with a Bachelor's degree in Nursing, preferred.
  • 1+ year of supervised clinical experience as a registered nurse.
  • Excellent verbal and written communication skills.
  • Strong computer skills and proficiency in electronic medical records.
  • Valid driver's license and reliable transportation.
