Job Summary

We are seeking a skilled RN Transition Coordinator to join our team at Henry Ford Health System. The successful candidate will be responsible for ensuring a smooth transition of patients from the inpatient setting to home care, working closely with hospital case managers and healthcare providers to coordinate care and services.

Key Responsibilities

  • Conduct thorough assessments of patients to determine their appropriateness for home care services
  • Collaborate with healthcare providers to develop and implement individualized care plans
  • Coordinate with hospital case managers to ensure a seamless transition of care
  • Provide education and support to patients and families to ensure a successful transition to home care
  • Stay up-to-date on knowledge of Henry Ford Health System's criteria, Medicare conditions of participation, and CHAP guidelines

Requirements

  • Registered Nurse licensure in the State of Michigan
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Knowledge of home care services and healthcare regulations
