Clinical Nurse Coordinator

At PACE Southeast Michigan, we are seeking a highly skilled and experienced Clinic RN Coordinator to join our team. As a key member of our Ambulatory Care team, you will be responsible for ensuring the smooth operation of our medical clinics.
Key Responsibilities:- Collaborate with physicians and nurse practitioners to optimize clinical flow and ensure high-quality patient care.
- Develop and implement daily workflows, delegating tasks to licensed practical nurses, medical assistants, and scheduling clerks as needed.
- Assess and triage patients in the clinic setting, providing education and resources to support their care.
- Supervise and mentor licensed practical nurses and medical assistants to ensure they possess the necessary clinical skills and competencies.
- Work closely with the manager to ensure adherence to quality initiatives and organizational standards.
- Ensure accurate and timely documentation of clinical activities in patient medical records.
- Collaborate with RN case managers and interdisciplinary team members to ensure seamless transitions of care and effective communication.
- Participate in ongoing education and professional development to enhance clinical skills and knowledge.
- Bachelor of Science in Nursing (BSN) degree preferred.
- At least one year of experience working with the frail elderly population.
- Five years of management experience preferred.
- Strong leadership and interpersonal skills, with the ability to motivate and direct staff.
- Excellent communication and documentation skills, with the ability to understand and standardize processes related to quality indicators.
- Ability to embody our culture traits of caring, belonging, honesty, evolution, fun, excellence, and gratitude.
- Possible exposure to communicable diseases.
- Position requires the ability to drive between locations on a daily basis as needed.
