Position Overview

The Healthcare Patient Support Specialist plays an essential role at Mary's Center by serving as the primary point of contact for patients and visitors, ensuring a welcoming and efficient experience. Bilingual candidates are highly preferred.

Reporting Structure

This position reports directly to the Patient Care Navigator Manager at the assigned location.

Key Responsibilities

To excel in this role, the individual must effectively perform the following essential duties:

  • Welcome and assist patients, visitors, and other stakeholders at Mary's Center.
  • Promote a positive and efficient office atmosphere by exemplifying professional behavior and conduct.
  • Convey important information to patients and the clinical team.
  • Comply with all confidentiality and HIPAA regulations set forth by Mary's Center.
  • Collaborate as part of the clinical team to enhance quality and health outcomes.
  • Coordinate patient care and services effectively.
  • Facilitate communication with external healthcare providers, specialists, and hospital services.
  • Assist in enrolling patients into the care team.
  • Gather socio-demographic data for population health management.
  • Engage in patient education initiatives and projects.
  • Collect and monitor data necessary for patient care and health outcomes reporting.
  • Implement clinical surveys and questionnaires to prepare for patient care.
  • Evaluate care affordability and support enrollment in insurance or discount programs.
  • Organize and maintain patient records for clinical care and public health.
    • Coordinate and share public health information with relevant health departments, including reportable infections and vaccinations.
    • Transfer clinical care information with other providers manually and through health information exchanges.
  • Prepare and maintain the patient care area.
  • Perform additional duties as assigned by supervisors.

Qualifications

The following qualifications are representative of the knowledge, skills, and abilities required. Reasonable accommodations may be provided to enable individuals with disabilities to perform essential functions.

Education and Experience - A minimum of a High School Diploma or GED equivalent is required. Previous experience in a medical office setting as a receptionist is preferred.

Physical Requirements - Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk, or hear; must be able to lift objects weighing up to twenty-five (25) pounds.

Work Environment - Primarily in a standard front office setting with a quiet to moderate noise level. May encounter extreme cold, heat, and humidity due to outdoor weather conditions.
