Job Overview

We are seeking a skilled Youth Transition Specialist to join our team at Greater Cincinnati Behavioral Health Services.

About the Role

This is a full-time position that offers a competitive salary of approximately $50,000 per year, with opportunities for productivity bonuses up to $10,000 annually. As a Youth Transition Specialist, you will be responsible for providing Community Psychiatric Support Treatment (CPST) services and Therapeutic Behavioral Services (TBS) to youths and young adults with severe mental illness in an outpatient facility and in the community.

Responsibilities
  1. Initiates, coordinates, and monitors evaluations necessary to identify each client's needs in areas to include: living arrangements, financial issues, benefit status, legal status, physical/psychiatric health, activities of daily living, social supports, leisure and recreation
  2. By assertive outreach, attempts to engage the client in identifying and expressing his/her own needs, offers services to assist in meeting these needs, and re-assesses outcomes with the client on a regular basis
  3. Gains knowledge of the following: mental health treatment and basic needs resources, informal and formal linkages with agencies that provide services, the client's use of services, and applicable costs of use, and the opportunities for alternative resources
Requirements
  • Masters degree in mental health, vocational training, education, social work, counseling
  • OR BSW (Bachelor of Social Work), Bachelor in psychology, criminal justice, or a related human services field plus two years of specialized education, training, or experience in one of the specified fields of study
  • Experience in social services and behavioral health preferred but not required
