Job Summary

We are seeking a skilled Care Manager to join our team at Martin Luther King, Jr. Community Hospital. This is a challenging and rewarding role that requires strong communication skills, attention to detail, and the ability to work collaboratively with various stakeholders.

About the Role
  • This position involves managing the continuum of care for assigned patients from admission through discharge, ensuring seamless transitions and optimal outcomes.
  • The successful candidate will be responsible for collaborating with the interdisciplinary team, physician, patient, and family to develop and implement individualized care plans.
  • A key aspect of this role is communicating relevant information regarding health plan benefits, target length of stay, and estimated discharge readiness.
  • In addition, the Care Manager will document all team, physician, and patient/family communication and concerns pertaining to coordination of care and services.
Requirements

To be considered for this position, applicants must possess:

  • A minimum of one (1) to three (3) years of hospital or related experience, with a focus on case management/coordination.
  • An Associate degree in nursing or Bachelor of Science degree in nursing.
  • Current California Nursing license, Basic Life Support (BLS), and Certification in Case Management (preferred).
  • Bilingual language skills (Spanish) are a plus.
Salary and Benefits

We offer a competitive salary range of $$65,000 - $85,000 per year, depending on experience. Our comprehensive benefits package includes medical, dental, and vision insurance, 401(k) matching, and paid time off.
