Job Overview:

Affordable Living for the Aging (ALA) seeks an experienced Care Manager for Seniors to join our team. As a Care Manager, you will play a vital role in supporting our organization's mission to eradicate senior homelessness and early institutionalization.

Job Description:

We are looking for a skilled and compassionate Care Manager who can deliver intensive case managing services to formerly homeless chronically homeless individuals. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with diverse populations.

Key Responsibilities:
  • Develop and implement comprehensive care plans for clients.
  • Provide linkages to health, mental health, and substance use disorder services as needed.
  • Assist clients with daily living skills, budgeting, and money management.
  • Crisis intervention and eviction prevention counseling.
  • Collaborate with property management staff and other stakeholders to ensure effective support for clients.
Requirements:
  • Bachelor's or master's degree in social work or a related field is preferred.
  • Minimum two years of experience working with mentally ill, chronically homeless individuals.
  • Strong understanding of managed care organizations, Medicare, and Medi-Cal.
  • Proven ability to work effectively with diverse populations.
  • CHAMP experience preferred.
Benefits:

This role offers an estimated annual salary of $60,000 - $70,000, plus benefits including health, dental, and vision insurance, paid time off, and opportunities for professional growth and development.
