The estimated salary range for this role is $72,000 - $76,000 per year. Mileage reimbursement is also available for travel to member locations within designated areas.

About the Role

The Lead Care Manager plays a vital role in overseeing case management for members and their families. This position requires exceptional communication skills, ability to work independently, and problem-solving capabilities.

Key Responsibilities:
  • Conduct comprehensive assessments to determine physical, emotional, and social needs of members
  • Develop individualized care plans based on assessment findings, considering medical history, preferences, and specific needs
  • Coordinate and facilitate communication between healthcare providers, social workers, therapists, and other members of the care team to ensure a comprehensive approach to care
  • Monitor progress of members and update care plans as needed per policy and compliance requirements
Requirements:
  • Associate degree and 2 years of healthcare or care coordination experience
  • Current and valid Driver's License
  • Proof of auto insurance
  • Current BLS certification from the American Heart Association upon start date
