POSITION OVERVIEW: We are seeking a dedicated Registered Nurse to provide compassionate care for patients in hospice and palliative settings. This role involves making home visits to deliver nursing services, ensuring that patients receive the highest quality of care.
EDUCATION AND CERTIFICATION: Candidates must have completed an NLN-accredited nursing program, holding either an associate or bachelor’s degree. A valid RN license in Connecticut is mandatory.
- Minimum of 1 year of recent experience in medical or surgical nursing.
- Experience in community health is advantageous.
KEY RESPONSIBILITIES: The Registered Nurse will provide intermittent nursing services to patients and their families, adhering to both state and federal regulations. Responsibilities include:
- Conducting comprehensive physical and psychosocial assessments.
- Developing and implementing individualized care plans.
- Referring patients to necessary therapies.
- Delivering education to patients and their families.
Additionally, the nurse will manage assignments efficiently, ensuring accurate and timely documentation, and responding appropriately to high-risk situations. Continuous professional development is encouraged through in-service training and self-directed learning.
