Hospice Music Therapist Job Description

Masonicare Home Health & Hospice is seeking a skilled Music Therapist to join our team in the Old Saybrook, CT area. As a Music Therapist, you will provide direct services to hospice and palliative care patients and their families in homes and inpatient and outpatient units.

Key Responsibilities:
  • Assess patients of all ages to identify needs and provide interventions that meet individual needs.
  • Collect data in collaboration with the patient and family, and document findings on the patient record.
  • Plan and implement care for patients based on assessed needs, ensuring individualized care.
  • Provide and document direct patient care according to the Care Plan, within set target dates, and according to all departmental policies.
  • Evaluate patient progress, take prompt action when changes in condition are noted, and terminate care when goals of management are attained.
Requirements:
  • Minimum of a Bachelor's degree; Masters preferred.
  • Graduate of an approved music therapy program; Board Certified as a Music Therapist.
  • A minimum of two years experience and work in the area of end of life issues preferred.
  • Maintains current Drivers' License and auto insurance.
  • Possess reliable transportation.
Competencies:
  • Accomplished in various instruments and in voice.
  • Demonstrates knowledge of composition and music theory, including skills in improvising and arranging.
  • Demonstrates training in assessment, treatment planning, and evaluation.
  • Demonstrates a commitment to recognized professional ethics and standards of practice.
  • Demonstrates ability to frequently concentrate on fine detail without interruption.
