Key Responsibilities:

  • Provide comprehensive health and psychosocial evaluations to identify residents' needs and develop effective care plans.
  • Monitor residents' conditions, health promotion, and prevention coordination of services to ensure optimal care.
  • Collaborate with our interdisciplinary team to ensure seamless care delivery and resident satisfaction.
  • Rotate weekend shifts to ensure 24/7 coverage and meet the needs of our residents.

Requirements:

  • Two years of LPN experience required.
  • Two years of experience in assisted living or a nursing home setting.
  • Licensed Practical Nurse license in good standing.
