- Utilize the patient-centered care model to ensure high-quality patient outcomes.
- Collaborate with interdisciplinary teams to coordinate patient care.
- Document patient care and progress in a timely and accurate manner.
- Current license to practice as a registered nurse in the state of Alaska.
- Basic Life Support Certification.
- One year of recent clinical nursing experience.
- Valid Alaska driver's license and vehicle liability insurance.
South Peninsula Hospital is a combined 22-bed critical access hospital and 28-bed Long Term Care facility, located in the heart of Homer, Alaska. The Home Health Program provides community-based services within the service area of South Peninsula Hospital. The service area extends from Ninilchik to Kachemak Selo at the head of Kachemak Bay.
As a Home Health RN/Case Manager, you will occasionally travel outside the service area per contractual agreement with insurance, drug companies, and other agencies. Travel in inclement weather is occasionally necessary.
