Caregiver Support Specialist

Caregivers at Hearts and Hands of Care provide essential support services to individuals with varying needs in their homes and communities. Key responsibilities include maintaining a safe environment, working towards individualized goals, and documenting daily activities. This role requires a strong focus on customer service, effective communication, and teamwork.
Key Responsibilities:
- Manage the high needs of service recipients and their families.
- Meet productivity standards and implement the Service Plan.
- Document comprehensive notes, contact information, and other relevant data.
- Promote a strengths-based approach to empower individuals to reach desired outcomes.
- Engage with children, individuals, and support networks within the Framework.
- Develop individualized Service Plans with collaboration from family and team members.
- Establish crisis and safety plans as needed.
- Monitor service delivery to ensure alignment with Service Plan goals.
- Maintain knowledge of community services and supports.
- Coordinate transportation for service recipients.
- Interact professionally with stakeholders.
Requirements:
- Must be at least 18 years old.
- Meet all requirements for the position as outlined in 7 AAC 125.010 – 7 AAC 125.199.
- Pass a Criminal Background Check.
- Not have been denied a healthcare provider license or certification for patient services.
- Be able to independently assist recipients with specific ADLs.
- Attend training and obtain First Aid certification.
- Attend Mandt training.
- Have confirmed service experience in the industry, preferably with advanced training in behavioral, medical, or caregiver services.
- Be reliable and dependable.
Work Environment:
- Indoors in a home setting and/or in a community setting.
- Kenai Peninsula: Homer, Kenai, Nikiski, Clam Gulch, Soldotna, Sterling, and Seward.
