Job Title: RN - Home Health Case Manager

South Peninsula Hospital is seeking a skilled RN - Home Health Case Manager to join our team. As a key member of our Home Health Services department, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Responsibilities:
  • Develop and implement individualized care plans for patients with complex medical needs.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop strategies to address these needs.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
  • Provide education and support to patients and their families to promote self-care and independence.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • Current RN licensure in the state of Alaska.
  • Basic Life Support certification.
  • One year of recent clinical nursing experience.
  • A valid Alaska driver's license and vehicle liability insurance.
  • Excellent communication and interpersonal skills.
