LHC Group is seeking a skilled Home Health Physical Therapy Assistant to join our team in Frederick, Maryland.

About the Role:
  • Perform treatment and provide exercise instruction and patient education aimed at improving/enhancing the patient's well-being.
  • Follow established plan of care and make recommendations for changes to the supervising Physical Therapist.
  • Instruct patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.
  • Observe, record, and report patient response to treatment and changes in patient condition to the supervising PT, nurse supervisor, and/or physician.
Responsibilities:
  • Participate in staff conferences and committees as requested by the agency.
  • Adhere to LHC Therapy Standards of Practice in provision of services to all patients.
  • Participate in a weekend rotation to provide services for their respective agency and other nearby LHC agencies as needed.
Requirements:
  • Current licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
  • Current CPR certification is required.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
