Job OverviewThis is an exciting opportunity for a skilled Physical Therapist to join our team in Queens, NY. As a Home Care Physical Therapist, you will play a vital role in delivering high-quality patient care and promoting independence.
Key Responsibilities:- Develop and implement individualized physical therapy plans to cater to the unique needs of each patient.
- Evaluate patient progress, identify areas for improvement, and make data-driven decisions to optimize care outcomes.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Demonstrated expertise as a physical therapist with at least 1 year of experience in home care settings.
- Valid licensure to practice physical therapy in the state of New York.
- Possession of a graduate degree or higher in physical therapy.
- Excellent communication, organizational, and time management skills.
- Bilingual skills are a plus.
