Primary Care Provider

We are seeking a skilled Primary Care Provider to join our HouseCalls team in Oregon. This dynamic role offers an exceptional opportunity to provide annual clinical assessments to patients in the comfort of their homes, promoting preventive care and improving health outcomes.
The ideal candidate will possess strong communication skills, empathy, and a passion for delivering high-quality patient-centered care. As a member of our integrated medical team, you will work collaboratively with other healthcare professionals to coordinate follow-up care, address health risks, and enhance patient well-being.
A competitive salary of $90,000 - $120,000 per year is offered, along with a $30,000 sign-on bonus for individuals who have not previously participated in this program.
Key Responsibilities:
- Conduct comprehensive home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams.
- Perform evidence-based practice screenings and point-of-care testing (as appropriate).
- Identify diagnoses and develop care management plans to address health gaps and promote active medical management.
- Communicate findings to inform primary care physicians and facilitate coordinated care.
- Respond to urgent and emergent situations, intervening as necessary to ensure patient safety.
- Educate patients on disease processes, medications, and compliance strategies.
Requirements:
- Active NP or PA license or ability to obtain by start date, with unencumbered licensure.
- ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or equivalent.
- Ability to complete physical requirements of the job, including lifting 30-pound bags and navigating various dwelling conditions.
- Reliable transportation for home visit assessments.
Preferred Qualifications:
- 1+ years of clinical experience in a relevant setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Ability to travel throughout the state, up to 5% of the time, for business needs.
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
