Job Summary:The successful candidate will provide medication therapy management services in collaboration with other healthcare providers to ensure safe, effective, and efficient drug therapy. The Clinical Pharmacist will be responsible for participating in interprofessional patient care activities, medication reconciliation, prescribing and managing medication therapy, medication therapy monitoring, review of medication orders, patient education, and providing drug information to other healthcare professionals.
Responsibilities:- Provide medication therapy management services in collaboration with other healthcare providers.
- Participate in interprofessional patient care activities, medication reconciliation, prescribing and managing medication therapy, medication therapy monitoring, review of medication orders, patient education, and providing drug information to other healthcare professionals.
- Collaborate with other healthcare providers to ensure safe, effective, and efficient drug therapy.
- Develop and implement medication therapy plans in collaboration with other healthcare providers.
- Monitor and evaluate the effectiveness of medication therapy plans.
- Provide education and training to other healthcare professionals on medication therapy management.
- Current, active, valid, unrestricted license for the practice of pharmacy in at least one state or territory in the United States.
- Post-baccalaureate or entry-level Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy.
- At least one of the following: Post-Graduate Year 1 (PGY1) Pharmacy Practice Residency from a program accredited by the American Society of Health-Systems Pharmacists (ASHP), Board certification in one or more specialties recognized by the Board of Pharmacy Specialties (BPS), or minimum of 3 years of post-graduate experience as a clinical pharmacist with at least 50% of time spent in direct patient care-related clinical activities.
- Minimum of one year of successful clinical practice within the past three years.
- Ability to read, write, speak, and understand English well enough to effectively communicate with all patients and other healthcare providers.
- Current certification in Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS).
- Ability to demonstrate knowledge and skills necessary to provide care appropriate to any age-related needs of patients.
