Job Description

We are seeking a skilled Professional Nursing Care Specialist to join our team. As a key member of our home health team, you will provide high-quality nursing care to patients in their homes, promoting independence, dignity, and comfort.

Your primary responsibilities will include:

  • Conducting comprehensive assessments of patients' physical and emotional needs
  • Developing personalized plans of care to address specific needs and goals
  • Implementing and evaluating the effectiveness of care plans

Requirements

To be successful in this role, you will need:

  • Associate's degree in nursing from an accredited program (Bachelor's degree preferred)
