Senior Community Care - A New Approach to Healthcare

As a Nurse Practitioner or Physician Assistant, you'll play a vital role in our mission to improve access to care and enhance the overall health and well-being of our patients.

Key Responsibilities:
  • Primary Care Delivery: Provide high-quality, cost-effective care to assigned members, managing both medical and behavioral conditions effectively.
  • Care Coordination: Assess medical necessity and coordinate care as members transition through different levels of care and care settings.
  • Program Enhancement: Regularly communicate with internal and external parties, promote the CCM program, and develop new and improved methods to address concerns and issues.
Requirements:
  • Certified Nurse Practitioner through a national board
  • Active and unrestricted license in the state of practice
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire
  • Access to reliable transportation
  • Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions
  • Availability to work 24 hours per month, with expectations of 16 hours during off-hours
Preferred Qualifications:
  • 1+ years of hands-on post-grad experience in Long Term Care
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end-of-life discussions
  • Proficient computer skills and ability to document medical information
