Key Responsibilities- Coordinate Internal and External Referrals: Work closely with medical and social service providers to facilitate referrals and ensure seamless transitions of care.
- Utilize SBIRT Model: Implement the Screening, Brief Intervention, and Referral to Treatment model to identify and address mental health and substance use issues.
- Assist with Goal Setting: Collaborate with individuals and families to identify and prioritize goals, and develop time-limited care coordination plans to achieve success.
- Provide Care Coordination and Resource Connection: Connect individuals and families with community resources and services to support their needs.
- Establish Relationships with Community Partners: Build and maintain relationships with community teams, providers, and agencies to ensure comprehensive care coordination.
- Complete Periodic Reports: Submit reports as required by the agency and state to ensure compliance and accountability.
- Maintain Accurate Clinical Documentation: Ensure timely and accurate documentation in the Electronic Medical Record to support care coordination efforts.
The Care Coordinator - Behavioral Health Specialist is an integral member of the HCRS team, providing care coordination, case management, and resource connection services. This role requires collaboration with other HCRS providers, community partners, and outside agencies to ensure comprehensive care coordination.
Key Knowledge, Skills, and Abilities- Engage, Empathize, and Show Compassion: Demonstrate a strong ability to engage and empathize with community members, and show compassion in all interactions.
- Knowledge of Developmental Trauma and Attachment Issues: Possess knowledge of developmental trauma and attachment issues, and their impact on an individual's quality of life.
- Knowledge of Mental Health, Substance Use, and Behavioral Issues: Have knowledge of mental health, substance use, and behavioral issues that impact an individual's quality of life, particularly within a culture of poverty.
- Knowledge of Community Resources: Possess knowledge of community resources and services available to support individuals and families.
- Healthy Communication Skills: Demonstrate healthy communication skills, including honest discussions, feedback, and exchanges.
- Intermediate or Advanced Computer Skills: Possess intermediate or advanced computer skills to effectively utilize electronic medical records and other software systems.
- Ability to Work Well with a Team: Demonstrate the ability to work well with a team, and demonstrate flexibility when consumer, program, and/or agency needs require it.
