Requirements
  • Doctor of Medicine or Doctor of Osteopathic Medicine degree from a school in the United States or Canada
  • Permanent, full, and unrestricted license to practice medicine in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States
  • Current/active board certification or be board eligible by the American Board of Medical Specialties in at least one of the following primary care disciplines: Family Medicine, Internal Medicine, Pediatrics, Obstetrics/Gynecology, Preventive Medicine, Psychiatry, Geriatrics, Emergency Medicine, or Anesthesiology
