Licensed Practical Nurse - Community Care Center

Work Schedule: Full-time, part-time, and on-call positions available.

Role Summary:

The Licensed Practical Nurse (LPN) is responsible for delivering exceptional care to residents in the nursing facility and their homes, under the supervision of a Nurse Case Manager or Clinical Supervisor. This role adheres to established nursing protocols and complies with the Nurse Practice Act, ensuring all activities align with relevant State and Federal regulations.

Qualifications Required:
  • Educational background that meets certification, licensing, or registration standards.
  • Current Oregon Licensed Practical Nurse license upon hire.
  • National Provider BLS certification from the American Heart Association within 30 days of hire.
  • Driving may be required; proof of valid driver's license and auto insurance will be necessary upon request.
  • A minimum of 2 years of patient care experience in a skilled nursing or medical clinic setting.
Preferred Qualifications:
  • Experience in Acute Care, Home Health, or Public Health nursing.
