Job Summary

Bloom at Kokomo is seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. Key Responsibilities
  • Assessing the health status of patients and documenting their medical history

  • Collaborating with the healthcare team to create individualized care plans for patients
  • Providing basic patient care, such as bathing, feeding, and dressing
  • Monitoring and recording vital signs, including blood pressure, heart rate, and temperature
  • Educating patients and their families about healthcare procedures, medications, and post-discharge care
  • Ensuring accurate and timely documentation of patient care in electronic medical records
  • Responding to emergency situations and providing immediate medical assistance when necessary
  • Collaborating with other healthcare professionals to maintain a safe and clean environment for patients
Requirements
  • Valid LPN license in [state of employment]
