Responsibilities:
- Provide comprehensive nursing care to patients, including physical assessments, monitoring vital signs, and administering medications as per physician's orders.
- Collaborate with the interdisciplinary healthcare team to develop and implement individualized patient care plans.
- Ensure accurate and timely documentation of patient information, treatment plans, and progress notes.
- Monitor and assess patient conditions, promptly identifying any changes or deterioration in order to initiate appropriate interventions.
- Educate patients and their families on health-related topics, including disease management, medication administration, and lifestyle modifications.
- Assist physicians and registered nurses during various medical procedures, including wound care, treatments, and examinations.
- Respond to emergencies and provide immediate interventions to stabilize patients.
- Maintain a clean and safe environment for patients, adhering to infection control guidelines and promoting a culture of patient safety.
- Administer intravenous fluids and medications as per physician orders, maintaining proper infusion techniques and monitoring for any adverse reactions.
- Collaborate with other healthcare professionals and community resources to ensure continuity of care and seamless transitions for patients.
Requirements:
- Valid and active LPN license in the state of employment.
- Graduate from an accredited practical nursing program.
- Proven experience working as an LPN in a healthcare setting.
- Knowledge of nursing principles, protocols, and procedures.
- Ability to multitask, prioritize, and work efficiently in a fast-paced environment.
- Strong communication and interpersonal skills, with the ability to establish rapport with patients, families, and healthcare team members.
