Patient Services Coordinator LPN Home Health Full Time

As a Patient Services Coordinator LPN at CenterWell Home Health, you will play a vital role in ensuring the seamless delivery of high-quality home health services to our patients. This position requires a unique blend of clinical expertise, exceptional communication skills, and a passion for delivering exceptional patient care.
Key Responsibilities- Scheduling and Coordination: Schedule visits and communicate with field staff, patients, physicians, and other stakeholders to maintain proper care coordination and continuity of care.
- Office and Staff Management: Assist with day-to-day office and staff management, including managing schedules for all patients.
- Infection Control and Compliance: Initiate infection control forms as needed and ensure compliance with regulatory requirements.
- Client Hospitalization Log: Maintain the client hospitalization log, including entering coordination notes and sending electronic logs to relevant staff.
- Staff Scheduling and Productivity: Ensure staff are scheduled for skilled nurse/injection visits and perform initial reviews of weekly schedules for productivity and geographic issues.
- Clinical Support: Receive lab reports, assess for normality, and fax copies to doctors and case managers.
- Experience: At least 1 year of home health experience.
- Certifications: Coding certification is preferred.
- Skills: Excellent communication skills, ability to interact well with diverse individuals, strong organizational skills, and ability to manage and prioritize multiple assignments.
