- Collaborate with our interdisciplinary team, including nurses, occupational therapists, and social workers, to ensure comprehensive care and support for our patients.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination and transitions.
- Document patient progress and outcomes accurately and thoroughly, using standardized tools and templates.
- Participate in ongoing education and training to stay current with best practices and evidence-based guidelines in physical therapy.
- Master's degree in Physical Therapy from an accredited program.
- Current and unrestricted Physical Therapy license in the state of practice.
- Minimum of one year of experience in home health or similar setting.
- Strong organizational and communication skills, with ability to work independently and as part of a team.
- Valid driver's license and reliable transportation.
