Community Support Specialist

Pathlight Group is seeking a dedicated and compassionate individual to join our team as a Community Companion. In this role, you will have the opportunity to make a meaningful difference in the lives of individuals with disabilities, supporting them in their daily lives and helping them to achieve their goals.
Key Responsibilities:
- Engage with and support individuals in social and recreational activities, promoting their independence and well-being.
- Assist with daily tasks and routines, such as transportation and meal preparation.
- Develop and implement personalized support plans, working closely with individuals and their families to ensure their needs are met.
- Provide emotional support and companionship, helping individuals to build confidence and self-esteem.
- Collaborate with other support staff and professionals to ensure a comprehensive and coordinated approach to support.
Requirements:
- High school diploma or equivalent.
- 2 years of experience working with individuals with disabilities, preferably in a support or care role.
- Valid driver's license and reliable transportation.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
