- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Coordinate and manage the delivery of care services, including medical, equipment, and pharmaceutical needs.
- Act as a liaison between patients, families, and healthcare providers to ensure seamless care coordination.
- Provide education and support to patients and families on disease management, self-care techniques, and prevention strategies.
- Document patient care and progress in accordance with regulatory requirements and company policies.
- Collaborate with interdisciplinary teams to coordinate care, problem-solve, and exchange information.
- Participate in ongoing education and training to enhance clinical expertise and stay current with industry developments.
- Graduate of an accredited Diploma, Associate Degree, or Baccalaureate School of Nursing.
- Current licensure by the Georgia State Board of Nursing and a valid Driver's License.
- Minimum two to three years of hospice experience, with a strong preference for experience in critical care, medical-surgical nursing, hospice, oncology, or community health nursing.
- Knowledge of Medicare, Medicaid, and other funding source regulations as applied to hospice care services.
- Ability to lift up to 50 pounds and move patients as needed.
- CHPN certification.
- Experience with electronic medical records and other healthcare software systems.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
This role requires extensive travel and working in patients' homes in various conditions and environments. The highest risk is in terms of personal safety regarding on-road travel.
