Nephrology Physician Assistant or Nurse Practitioner

CentraCare Health is seeking a highly skilled Nephrology Advanced Practice Provider to join their collaborative team. This is a full-time opportunity with a competitive salary and benefits package.
Position OverviewThe Nephrology Advanced Practice Provider will practice in a state-of-the-art kidney program, providing patient care to outreach/dialysis sites and rounding at St. Cloud hospital. The ideal candidate will have experience in nephrology and/or dialysis, with strong organizational management, leadership, and communication skills.
Responsibilities- Provide patient care to patients with end-stage renal disease, chronic kidney disease, and acute kidney injuries
- Manage anemia, hypertension, and chronic kidney disease education
- Collaborate with the nephrology team to provide comprehensive care
- Develop and implement treatment plans
- Communicate effectively with patients, families, and healthcare teams
- Must be a Nurse Practitioner or Physician Assistant licensed in Minnesota
- Successful completion of a formal program approved by a national accrediting agency and certified by a national certification organization recognized by the Minnesota State
- Previous experience in nephrology and/or dialysis preferred
- Basic Life Support (BLS) certification and advanced practice prescriptive privileges required
