Nephrology Physician Assistant or Nurse Practitioner

CentraCare Health System is seeking a highly skilled Nephrology Advanced Practice Provider to join their dynamic team. This is an exceptional opportunity to work in a state-of-the-art kidney program, providing comprehensive patient care to patients with end-stage renal disease, chronic kidney disease, and acute kidney injuries.
Key Responsibilities:- Provide patient care to patients with kidney disease, including dialysis, chronic kidney disease management, and nephrology-related diagnoses.
- Develop and implement individualized treatment plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Must be a licensed Nurse Practitioner or Physician Assistant in Minnesota.
- Successful completion of a formal program approved by a national accrediting agency and certification by a national certification organization recognized by the Minnesota State.
- Previous Nephrology and/or dialysis experience is preferred.
- Basic Life Support (BLS) certification and advanced practice prescriptive privileges are required.
