About the Position

The Children's Home of Reading Youth & Family Services is seeking a highly skilled and compassionate Residential Case Manager to join our team in Germantown. As a key member of our staff, you will play a critical role in ensuring the delivery of high-quality care and services to our clients.

Key Responsibilities

  • Maintain accurate and up-to-date client records, including medical information and individual service plans (ISPs).
  • Cordinate health care appointments and ISP meetings to ensure compliance with regulatory requirements.
  • Provide support and guidance to direct care staff on creating and adjusting behavioral goals.
  • Facilitate in-home visits and track visitation records.
  • Assist in medical appointment transportation when necessary.
  • Perform other job duties as assigned.

Qualifications

  • Bachelor's degree with a background in behavioral modification or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
