We are seeking a skilled and dedicated Care Coordinator Registered Nurse to join our team in the Austin Market Float Pool. This is a critical role that involves performing comprehensive patient assessments, developing personalized care plans, and implementing and evaluating evidence-based interventions.

Responsibilities:

  • Conduct thorough patient assessments at admission, including identifying potential health risks and prioritizing care needs
  • Create tailored discharge plans based on patient response to treatment, incorporating input from interdisciplinary teams
  • Diligently document outcomes of nursing interventions and patient responses, ensuring accurate and up-to-date records
  • Develop and provide educational materials to patients and families on effective self-care strategies and treatment options
  • Administer medications and complete treatments according to hospital policy, adhering to strict safety protocols
  • Perform venipuncture and manage IV fluids and medications with precision and accuracy
  • Review new physician orders, verify their implementation, and notify relevant departments of changes or services required
  • Provide timely and accurate patient information reports to ensure seamless continuity of care

Requirements:

  • Minimum 2 years of experience in acute care nursing settings, with a strong understanding of hospital operations
  • Associate Degree in Nursing (ADN) from an accredited program, with a focus on delivering high-quality patient care
  • Current State of Texas Registered Nurse license or Compact Registered Nursing License; Texas license must be obtained within 90 days of hire
  • Basic Life Support (BLS) certification as per St. David's Healthcare policy, with ACLS certification required within 6 months
  • National Institutes of Health Stroke Scale (NIHSS) certification is mandatory, per St. David's Healthcare Policy, within 60 days of hire
