Key Responsibilities:- Develop and implement individualized care plans that meet the unique needs of each patient.
- Provide primary and specialty care to patients in their homes, focusing on symptom management and quality of life.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Conduct thorough assessments, develop and implement treatment plans, and provide ongoing care and support to patients and their families.
- Participate in quality improvement initiatives and contribute to the development of clinical practice guidelines and standards.
- Masters Degree in Nursing and certification as a Nurse Practitioner.
- 3+ years of clinical nursing experience, with at least 1 year in hospice and palliative care.
- Current licensure as a Nurse Practitioner in the state of practice and ability to maintain licensure in accordance with applicable laws and regulations.
- Board Certification in Palliative Care is preferred.
- Ability to travel up to 50% as needed to support business and patient needs.
