We are seeking a compassionate and skilled Certified Nursing Assistant to join our Hospice team. As a CNA, you will provide high-quality care to patients in their homes, supporting their physical, emotional, and spiritual needs.

Responsibilities
  • Provide personal care services, including bathing, dressing, and grooming
  • Assist with daily living activities, such as meal preparation and medication management
  • Monitor and report changes in patient condition to the Case Manager
  • Support patients and families with emotional and spiritual needs
  • Collaborate with the hospice team to develop and implement individualized care plans
Requirements
  • Current certification as a Certified Nursing Assistant
  • Minimum of 6 months experience in a hospice or home health setting
  • Ability to read and follow written instructions and document care given
  • Strong communication and interpersonal skills
  • Ability to work with little direct supervision
