Job Summary:

Halifax Health Hospice is seeking a skilled and compassionate Hospice Care Navigator Leader to promote our health services in the community through educational inservices and liaison with physicians, healthcare facilities, and community groups. The ideal candidate will have a Bachelor's degree in a related field or equivalent years of experience, with knowledge of hospice and home healthcare philosophy, Medicare conditions of participation, and State of Florida regulations governing hospice and home healthcare.

 

Key Responsibilities:

  • Educate referral sources, including physicians, hospital case managers, nursing home staff, and assisted living facility staff, about skilled care and end-of-life care services offered by Halifax Health-Hospice.
  • Develop plans of care for referral sources and identify barriers to early referral.
  • Act as liaison between referral sources and clinical operations of the health system to ensure optimal customer satisfaction and patient outcomes.
  • Provide feedback to ensure customer satisfaction and patient outcomes.
  • Establish and maintain good working relationships with referral sources.

 

Requirements:

The ideal candidate will have a Bachelor's degree in a related field or equivalent years of experience. Knowledge of hospice and home healthcare philosophy, Medicare conditions of participation, and State of Florida regulations governing hospice and home healthcare is required. Healthcare experience and medical sales and marketing experience are preferred. The candidate must have good written, verbal, and public speaking skills and a valid Florida driver's license with a clear driving record.

 

Salary:

The salary for this position is estimated to be around $60,000 - $80,000 per year, depending on experience.
