About the Role

We are seeking a compassionate and skilled Registered Nurse to join our team as an On Call RN. This role will provide care to patients in their homes, promoting dignity, respect, and integrity throughout their journey.

Responsibilities
  • Provide high-quality patient care in a home setting, adhering to our organization's philosophy and mission.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Conduct thorough assessments, identify patient needs, and develop effective care strategies.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
Requirements
  • Graduate of an accredited school of nursing.
  • One to two years of recent acute care experience in an institutional setting.
  • One to two years of recent experience in home care.
  • Current licensure in the state of residence and a valid driver's license.
  • Excellent observation, verbal, and written communication skills.
  • Ability to lift, position, or transfer patients, as well as lift supplies and equipment.
  • Visual acuity and hearing to perform required nursing skills.
