Job Summary:

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Ann's Choice by Erickson Senior Living. As an LPN, you will play a vital role in providing high-quality clinical care to our residents, working collaboratively with our healthcare team to ensure the best possible outcomes.

Responsibilities:

• Develop and maintain relationships with residents and their families, providing exceptional care and support.
• Assess and evaluate resident clinical conditions, documenting findings and notifying providers as needed.
• Update and modify care plans to reflect changes in resident care and service needs.
• Supervise and lead care associates, ensuring the delivery of high-quality care.
• Administer medications accurately and timely, according to orders.
• Participate in meetings related to resident outcomes and care, contributing to the development of effective care plans.
• Respond to resident, family, or responsible party inquiries in a timely and professional manner.
• Participate in the review of clinical indicator reviews, including falls, wounds, nutrition, and antipsychotic use.

Requirements:

• Graduate of an approved Licensed Practical/Vocational Nursing program.
• Minimum of 1 year of experience in assisted living, home health, or long-term care preferred.
• Passion for caring for seniors and advocating for their needs.
• Experience with electronic medical records preferred.
