Key Responsibilities

This role involves delivering compassionate care to assigned residents in a secure and efficient manner, adhering to established standards, procedures, and personalized resident care plans.

  • Conduct thorough nursing assessments for all residents upon admission and continuously as per state and federal regulations, utilizing the nursing process (assessment, diagnosis, intervention, and evaluation).
  • Exhibit proficiency in clinical skills as outlined in the Competency Verification and Training Checklist.
  • Ensure that assigned residents receive prescribed medications, treatments, and procedures within specified timeframes, following appropriate infection control protocols.
  • Demonstrate understanding of the therapeutic effects and potential side effects of all administered medications.
  • Procure necessary equipment and supplies to facilitate care as required.
  • Adhere to procedures regarding pressure ulcers, dehydration, malnutrition, incontinence, pain management, and medication administration.
  • Accurately perform glucometer testing and control monitoring as per established procedures.
  • Implement tailored strategies to address behavioral issues and enhance residents' emotional well-being in accordance with their care plans.
  • Oversee resident care documentation, ensuring updates are made as necessary.
  • Utilize the Electronic Medical Record (EMR) system accurately, documenting all actions and observations related to changes in resident conditions.
  • Document all actions taken concerning admissions, discharges, transfers, or resident deaths, including incidents or accidents.
  • Participate in quality improvement initiatives, including audits and corrective actions as assigned.
  • May assist in completing designated sections of the MDS for Nursing and sign off on MDS forms to confirm completion.
  • Process physician orders and notify physicians promptly as needed.
  • Engage in rounds with physicians as required.
  • Attend or obtain resident reports prior to the start of shifts, sharing information and observing residents' conditions.
  • Respond promptly to resident call lights and assist with resident and family education.
  • Notify families of any changes in resident conditions and ensure timely follow-up on signs or symptoms of infections.

Qualifications

Must be a graduate of an accredited nursing program. Previous experience in a similar role is preferred. Maintain all required certifications and qualifications, along with relevant standards. Licensure for the state of practice is mandatory.

Possess knowledge of fundamental nursing skills and familiarity with the therapeutic effects and side effects of all medications administered.
