Job Summary

The Service Coordinator is a vital role in our organization, responsible for providing essential services and support to residents in our Affordable Housing Portfolio. This position requires a strong understanding of the needs of seniors and persons with disabilities, as well as excellent communication and interpersonal skills.

Key Responsibilities
  • Conduct needs-based assessments to identify resident needs and develop personalized service plans.
  • Provide case management services, including connecting residents with community resources and support services.
  • Collaborate with community partners to ensure residents have access to necessary services and support.
  • Facilitate health, wellness, and educational programs to promote resident well-being and community engagement.
  • Maintain accurate records and reports to ensure compliance with program requirements.
Requirements
  • Bachelor's degree in human services, social work, or a related field.
  • At least one year of experience working with seniors and persons with disabilities.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Proficiency in Microsoft Office and database management.
Preferred Qualifications
  • Mental health experience.
  • Bilingual or multilingual skills.
Working Conditions

This position works in an indoor office environment with occasional local travel required. Some out-of-state travel may be necessary for training and conferences.
