- Triages telephone calls and responds to patients and families, addressing questions and concerns within your scope of practice.
- Communicate effectively with healthcare teams and stakeholders to ensure seamless care coordination.
- Advocate for patients and families, ensuring access to necessary services and benefits.
- Recognize situations requiring referrals to Quality, Risk Management, or Patient and Family Relations, and discuss with leadership.
- Evaluate patient outcomes and identify opportunities for quality improvement.
- Collaborate with clinical research to coordinate care for patients on complex protocols.
A graduate of an accredited Nursing Program is required. A Bachelor's degree (or matriculated into a Bachelor's degree program with completion within a specified timeframe) with 3 years of clinical experience is also required. Recent acute care hospital experience is preferred.
Desired qualities and behaviors include a collaborative team player with strong communication skills, a systems approach to planning and problem-solving, creativity, innovation, risk-taking, autonomy, flexibility, and a commitment to professional growth.
Computer skills and licensure/certifications, including a Licensed Registered Nurse in New Hampshire and Basic Life Support (BLS) Certificate, are also required.
