Clinical Judgment and Decision-Making

As a Nurse Practitioner at Acadian Medical Center, you will utilize your clinical reasoning skills to conduct accurate assessments, identify patient and family needs, and develop, implement, and evaluate the nursing plan of care. You will prioritize patient and family needs, modify the plan to meet clinical outcomes, and ensure a safe and therapeutic environment.

Clinical Inquiry and Evidence-Based Practice

You will systematically evaluate the quality and effectiveness of nursing practice, including nursing-sensitive indicators, and support evidence-based practice changes through research utilization and experiential learning. You will participate in quality/performance improvement initiatives and contribute to the professional development of peers, colleagues, and others.

Caring Practices and Therapeutic Relationships

You will create a compassionate, supportive, safe, and therapeutic environment for patients, families, and staff. You will develop therapeutic relationships with patients and families, maintain appropriate boundaries, and manage both emotional and physical pain to promote comfort and healing.

Response to Diversity and Cultural Sensitivity

You will recognize, appreciate, and incorporate a patient's and family's unique differences, such as culture, spiritual beliefs, gender, race, ethnicity, lifestyle, socioeconomic status, age, and values, into an individualized plan of care.

Advocacy and Moral Agency

You will preserve and protect the confidentiality, autonomy, dignity, and rights of patients and families, represent their concerns, and create an individualized plan that accurately reflects patient and family values and goals.

Facilitation of Learning and Education

You will facilitate learning for patients and families, nursing staff, other members of the healthcare team, and community, integrating appropriate education throughout the continuum of care to help them participate and/or make informed decisions about their healthcare and treatments.

Collaboration and Teamwork

You will work collaboratively and interdependently with colleagues and community to develop and implement an integrated plan of care, open and sensitive to all team members' unique contributions, delegate tasks and care to appropriate staff, and ensure timely follow-up.

Systems Thinking and Problem-Solving

You will use strategies and available resources for problem-solving for patients, families, and staff, recognizing that resources are limited and considering factors related to safety, effectiveness, and efficiency in planning and delivering patient care.

Professionalism and Shared Governance

You will improve nursing practice and the work environment through participating in shared governance and decision-making processes, meaningfully recognizing the contributions of others, and participating in the nursing process from education and planning to evaluation.

Assessment and Evaluation

You will assess patient physical, psychological, social, spiritual, educational, developmental, and cultural and discharge planning needs, review patient history and physical with patient/family, and assure completion within an appropriate timeframe.

Plan of Care and Discharge Planning

You will formulate a plan of care, including the discharge plan, utilizing assessment data and patient, family, and health team input, and initiate a plan of care based on patient-specific needs, assessment data, and the medical plan of care.

Implementation and Coordination of Care

You will implement the plan of care through direct patient care, coordination, delegation, and supervision of the activities of the healthcare team, provide care based on physician orders and the nursing plan of care, and promote continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.

Evaluation and Documentation

You will evaluate the patient's and family's response to care and teaching, and revise the plan of care as needed, evaluate patient progress towards goals and expected outcomes in collaboration with other healthcare team members, and document assessment, planning, implementation, and evaluation in the patient record.

Best Evidence and Research-Based Practice

You will provide care based on the best evidence available and may participate in research activities within clinical practice, participate in unit or facility shared leadership, interact and participate in the education, role development, and orientation of facility personnel, patients, students, families, and visitors, and promote/support growth of others through precepting and mentoring when appropriate.
