Job Summary

Squirrel Hill Health Center is seeking a highly skilled Care Navigation Manager to join our team. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and navigation services to our patients.

Key Responsibilities
  • Supervise and manage the Care Navigation team, ensuring seamless patient care and navigation services.
  • Act as a liaison between patients, referral agencies, and healthcare providers, ensuring effective communication and coordination.
  • Provide case management services to patients, including goal setting and tracking progress.
  • Link patients with necessary social services, ensuring they receive comprehensive care.
  • Develop and maintain relationships with community partners and healthcare providers, identifying new opportunities for collaboration and referral pathways.
  • Coordinate insurance company authorizations and referrals, ensuring timely and efficient patient care.
Requirements
  • Bachelor's Degree in Social Work or public health related field required, MSW preferred.
  • Minimum of 1 year working in a supervisory capacity.
  • Experience working with diverse populations.
  • Bilingual a plus.
  • Excellent organization skills and ability to deal effectively with the public.
  • Skilled in using computer, printer, and fax machine.
  • Medical terminology, ICD-10 and CPT coding knowledge preferred.
