Key Responsibilities- Conduct thorough psychosocial assessments to identify patient needs and develop individualized treatment plans.
- Provide evidence-based behavioral health interventions, including individual and group therapy, to support patients and families.
- Collaborate with patients, families, and healthcare teams to develop comprehensive discharge plans that meet ongoing care needs.
- Assess and address social determinants of health, providing education, referrals, and resources as needed.
- Participate in emergency response efforts and provide guidance to other team members as needed.
- Supervise social work students and contribute to the development of new staff members.
- Master's degree in Social Work (MSW) from an accredited program.
- Minimum of 2 years of post-licensure work experience in a healthcare setting.
- NJ Licensed Clinical Social Worker (LCSW) certification.
- Excellent communication, interpersonal, and problem-solving skills.
- Proficiency in Microsoft Office and/or Google Suite platforms.
- Experience in home health, hospice, or palliative care settings.
- Ability to provide guidance and mentorship to other team members.
