We are seeking a highly skilled Clinical Social Worker to join our team at Kaiser Permanente. As a key member of our Behavioral Health department, you will be responsible for providing integrated and coordinated case management services for high-risk and high-utilizer populations in both inpatient and outpatient psychiatry and addiction medicine settings.
Key Responsibilities- Manage all facets of care and utilization of high-risk psychiatric and addiction medicine patients
- Provide assessment and evaluation of clinical status, environmental resources, mental and intellectual capabilities, and networking skills
- Plan and coordinate services based on assessment of short- and long-term needs
- Track referrals and outcomes for ongoing quality management
- Collaborate with Primary Care, Behavioral Health Care Services, and Health Care Providers to facilitate care coordination
- Provide referrals to other departments and facilitate follow-up services as needed
- Consult with families and other caregivers to provide education and resource information
- Link members to community services and establish working relationships with outside resources
- Provide individual and group therapy for chronic, high-utilizing patient populations
- Participate in Intensive Outpatient programs as a team member
- Master's degree in Social Work, Psychology, Sociology, or other field related to behavioral health
- Licensed Professional Clinical Counselor (California) or Licensed Marriage and Family Therapist (California) or Licensed Clinical Social Worker (California)
- Minimum two years of recent experience in case management and working with community resources
- Knowledge of psychiatry and addiction medicine patient populations in inpatient and outpatient settings
- Knowledge of complex systems of psychiatric care and substance abuse treatment at the community and county levels
