Position Overview:

We are in search of an entry-level Bilingual Community Engagement Coordinator to become part of a vibrant team focused on serving underserved communities. The Coordinator will play a vital role in delivering direct support and organizing care services for individuals. The ideal candidate should possess experience in direct patient support or relevant education, along with strong organizational and communication skills, complemented by an empathetic approach.

Key Responsibilities of the Bilingual Community Engagement Coordinator:

  • Facilitate patient care activities, which include scheduling appointments, coordinating referrals, and ensuring follow-up services.
  • Keep precise and current patient records within electronic medical record systems.
  • Engage with patients, healthcare professionals, and insurance providers to guarantee effective care coordination.
  • Offer physical, emotional, and behavioral support to individuals, encouraging their active participation in community activities while ensuring their personal safety.
  • Provide transportation using personal and company vehicles throughout the day to assist individuals in reaching their homes and community activity locations.
  • Inform patients about available resources and support services.
Qualifications for the Bilingual Community Engagement Coordinator:
  • Customer Service Focus: Proactively assist clients in a professional and friendly manner to enhance their experience by building relationships and understanding service quality standards.
  • Social Awareness: Recognize and understand the reactions of others, maintaining a positive demeanor even in challenging situations.
  • Confidentiality: Handle confidential and sensitive information with care and discretion.

Compensation:
  • $38,000-42,000 annually + Benefits
Requirements:
  • Associate's Degree required; Bachelor's degree preferred in a related field.
  • Prior experience in a patient support capacity is highly valued.
  • Familiarity with medical terminology is advantageous.
  • Bilingual proficiency in Spanish and English is essential.
