We are seeking a skilled LPN to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.
Key Responsibilities- Provide professional nursing services under the direction of a RN, in compliance with state and federal regulations, and CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Maintain accurate and detailed records of patient care, including visit notes and progress reports.
- Collaborate with other healthcare professionals to ensure seamless care coordination and patient outcomes.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
