Certified Nursing Support Specialist

The Certified Nursing Support Specialist will play a vital role in providing hands-on care to patients in their homes, nursing facilities, and other settings. This position requires a strong commitment to delivering high-quality care, attention to detail, and excellent communication skills.
Responsibilities- Conduct initial assessments of patients' needs and develop individualized care plans
- Deliver skilled nursing services, including wound care, catheter management, and ostomy care
- Collaborate with multidisciplinary teams to ensure seamless transitions of care
- Participate in ongoing education and training to enhance clinical knowledge and skills
- Document all patient interactions and care delivered in accordance with company policies and procedures
- Certified Nursing Assistant (CNA) certification; current CPR/BLS certification required
- Minimum 1 year experience in certified nursing assistant or related field; prior hospice experience a plus
- Ability to work independently and as part of a team; strong problem-solving and critical thinking skills
- Basic computer skills and familiarity with electronic medical records (EMR) systems
$38,500 - $48,000 per year, based on location and experience.
